Along with the above stated fact, we also provide a consent form. The client must tick the checkbox stating the following
“I confirm to have read in detail the consent form and hereby give consent to “Dental Radiology Solution” to provide me with their reporting services.”
If the checkbox is not ticked, we will not proceed with the services.
